1. When examined for the first time in the ER, the patient was alert and hemodynamically stable, and her temperature was 38.5o C. The physical examination at admission was normal except for a right bronchial breathing on chest auscultation.

Afin de caractériser avec plus de précision les différentes souches (1 du sujet contact et 3 des patientes atteintes) et d’établir leur homologie sur le plan moléculaire, on a expédié les échantillons au National Centre for Streptococcus, à Edmonton (Alberta).

2. RSV prophylaxis with palivizumab, if undertaken, should be initiated at the start of the RSV season and continued monthly until the end of the season. Palivizumab is not indicated for the inpatient treatment of established RSV infection. Children less than 2 years of age with hemodynamically significant cyanotic or acyanotic congenital heart disease (who require corrective surgery or are receiving cardiac medication for hemodynamic considerations) should be considered for monthly palivizumab prophylaxis during the winter season.

La prophylaxie au palivizumab devrait être mise en route, s'il y a lieu, au début de la saison de l'infection à VRS et poursuivie de mois en mois jusqu'à la fin de la saison. Le palivizumab n'est pas indiqué pour le traitement à l'hôpital d'une infection à VRS établie. On devrait envisager une prophylaxie mensuelle au palivizumab durant la saison hivernale dans le cas des enfants de moins de 2 ans qui présentent une cardiopathie congénitale cyanogène ou non cyanogène hémodynamiquement significative (qui ont besoin d'une chirurgie correctrice ou reçoivent des médicaments pour le cœur pour des raisons hémodynamiques).
