1. In patients with inhibitor titres above # BU or with high anamnestic response, the use of (activated) prothrombin complex concentrate (PCC) or recombinant activated factor # (rFVIIa) preparations has to be considered

Bei Patienten mit einem Hemmkörpertiter von mehr als # B.E. oder einem großen Faktor # Bedarf in der Anamnese muss die Gabe von (aktiviertem) Prothrombin-Komplex-Konzentrat (PPSB) oder von rekombinantem aktivierten Faktor # (rFVIIa) erwogen werden

2. Tests made before and after operation included Quick’s test, partial thromboplastin time (PTT), Thrombin time, fibrinogen (factor I), prothrombin (factor 11), and pro-accelerin (factor V), as well as WBC and RBC counts to exclude any disturbances due to intraoperative hemorrhage.

Prä- und postoperativ wurden Prothrombinzeit nach Quick, partielle Thromboplastinzeit (PTT), Thrombinzeit, Fibrinogen (Faktor I), Faktor II (Prothrombin) und Faktor V (Proaccelerin) untersucht. Dazu kamen Untersuchungen des roten und weißen Blutbildes, um Störeinflüsse durch intraoperative Blutungen auszuschließen.

3. Over the course of time the patient developed a further rise in temperature to 41.8°C, rhabdomyolysis with an increase of the CK to 10190U/L, liver damage with an AST of 3938U/L and an ALT with 3591U/L, coagulation disorder with a prothrombin time <12% and a decrease of the platelets to 22000/μL, hemolysis with a drop of hemoglobin to 6.7g/dL, pancreatitis with an amylase of 108U/L and a lipase of 1530U/L and a severe cerebellar dysfunction with dysarthria, tremor and ataxia.

Im weiteren Verlauf entwickelte die Patientin einen weiteren Temperaturanstieg auf 41,8°C, eine Rhabdomyolyse mit einem maximalen CK-Anstieg auf 10190U/l, eine Leberschädigung mit einer GOT von 3938U/l und einer GPT von 3591U/l, eine Gerinnungsstörung mit einem Absinken des Quick-Wertes auf &lt;12% und einem Thrombozytenabfall auf 22000/μl, eine Hämolyse mit einem Hb-Abfall auf 6,7g/dl, eine Pankreatitis mit einer Amylase von 108U/l und einer Lipase von 1530U/l sowie eine schwere cerebelläre Funktionsstörung mit Dysarthrie, Tremor und Ataxie.
