Primäre maligne Knochentumoren treten relativ selten auf.Die häufigsten Knochenläsionen sind das Osteosarkom,das Ewing-Sarkom,das Chondrosarkom,das Fibrosarkom, das maligne fibröse Histiozytom des Knochens,Riesenzelltumoren, aneurysmatische Knochenzysten und das Chordom.Im Allgemeinen gelten diese Knochentumoren als weitgehend strahlenresistent.Jedoch sollte dies nicht zu einem therapeutischen Nihilismus oder frustranen Resektionsversuchen bei inoperablen Tumoren führen,da gezeigt wurde, dass die Strahlentherapie auch bei diesen Tumoren gute palliative z.T. auch kurative Ergebnisse erzielen kann, wenn eine ausreichende Dosis und ein adäquates Volumen bestrahlt wird.Lediglich beim Ewing-Sarkom hat die Strahlentherapie einen festen Platz in der Primärbehandlung im Rahmen der multimodalen Therapiestrategie.
